"All About Me" Poster

We ask you to complete the "All About Me" poster that you received from the HELP team with the patient today or tomorrow so we can get to know them better and understand their preferences.

You can add anything else to the poster that helps us better understand the patient and their preferences.

Please contact the HELP trainer if you have not already received your "All-About-Me" poster and instructions for how to complete/hang the poster.

• As you fill in the boxes with the patient, feel free to make suggestions if the patient gets stuck.
• “My Name is” — Write in the name that the patient wants to be called.
• “I am from” — Write in where the patient is from – this can be either the town that they are living in right now or the state/country they are from (if they want us to know that).
• “I worked as” — Here, you can write in a previous profession or any hobbies they want to share.
• “Things that make me happy” — List anything that comes to mind here.
• “What is important to me in my life and health” — List anything that comes to mind here. If you get stuck, you can ask if they have a specific goal, like walking a certain distance
• “Things that stress me out” — List anything that is difficult for the patient and that is good for us to know. This could be an external stress such as bright light or something about their situation.
• “My favorite music” — List any music or other favorite activities.
• “I use Hearing Aids or Glasses” — please circle YES or NO
